I'm not able to see my custom performance counters appear in the "add metrics" window. I see the vales being reported in the WADPerformanceCountersTable but can't get to view them in the management portal. Only difference I see with your demo is the fact that my web role runs windows server 2012. Any ideas why they don't appear?

Looks like deleting the cloud service and recreating/republishing did the trick.
