I like the Notepad2 for the notepad part of the demo. I've seen Mike Taulty use notepad, in a few of his screencasts. The ability to scale the text up and include red line numbers seems like an easy way to add visual appeal.

Mike's a damn fine presenter. Just don't tell him I said that.
As for effort in presenting, I know mike rehearses for weeks. When I've done it I find that a new presentation takes about 2-3 weeks for an hour presentation, and then regiving it still takes about 4 hours to refresh yourself. And I'm never happy with the darned things.
