I was musing over the challenge of implementing Agile methodlogies in a somewhat difficult management environment recently, and thought this would be a good question for C9'ers:

Why is it that while we all know how good development should be done (numerous books, tools, experiences of others, etc.) we have such a difficult time implementing good teams?      

Even with a tool like VS2005, and the MSF Agile built right in, actually having your team get going is often plauged with difficulty, and results in failure. And by "team" I mean something more than 5 developers. This stuff is relatively easy to implement on small teams, or in small orgs where management and other stakeholder involvement is low.

What are your experiences? What tools have you used to help you do it right?
